USER AGREEMENT

A. ACCEPTANCE OF TERMS

Your access to the Garner 5th Ave Pharmacy mobile application and websites, including co-branded Garner 5th Ave Pharmacy-affiliate websites (collectively, the “Garner 5th Ave Pharmacy Digital Services”), is subject to the Garner 5th Ave Pharmacy Privacy Policy, HIPAA Privacy Notice, and these Terms of Use.

IF YOU DO NOT AGREE WITH ANY OF THESE TERMS, YOU ARE PROHIBITED FROM USING THE Garner 5th Ave Pharmacy DIGITAL SERVICES. BY USING THE Garner 5th Ave Pharmacy DIGITAL SERVICES, YOU AGREE TO THESE TERMS OF USE.

These Terms also govern your participation in any SMS or mobile communications programs offered by Garner 5th Ave Pharmacy. If you opt in to receive text messages from us, the Mobile Terms & Privacy Summary governs such messaging.
